An Introduction

My name is Dwight Baxter. As of June 26Th of 2009 I will have been a field service technician with Bradford-Scott Data Corp. for twenty years. I have continued to work for this same company because I like the work I do. I enjoy traveling to our many customer's offices. It is nice to visit with our clients who I consider friends to help resolve any problems they may have with their equipment. Bradford-Scott has evolved greatly in those twenty year. In the beginning our office was near Michigan Rd. and 86Th St. We are now in the Precedent Office Park at Keystone Ave. and 96Th St., a much better facility. When I was first hired we sold computer systems to medical practices, building material suppliers and construction companies. Several years ago the building materials and construction business was sold so we could concentrate on our medical software and hardware business. We added a credit union division a few years back that has done well, but a clear focus has always remained on our medical customers.
Today our medical division is selling Intergy from Sage. We changed to this product to meet the needs of our customers. EMR will soon not be an option. We researched EMR offering until we found a fully integrated system we could be confident in.
We currently have Pediatrics EMR, Orthopedics EMR, Psychiatric EMR, among others who are very happy with the new Intergy product.
Yes, the medical practice environment have changed. Electronic claims, electronic prescriptions, and electronic medical records are all here. If Bradford-Scott did not change to meet these new challenges we would not still be around.

Health Insurance Company Unfair to Customers?

I read an article that I find to be proof positive that insurance companies do not care about their customer, just money. A large insurance company, that does a big business in California, sent out increases to select customers of 39%. Most were individuals not part of groups. The company, when questioned, said the increase was due to the recession. They say many young healthy customers lost jobs and could no longer afford insurance coverage. This left them with a larger proportion of older unhealthy customers. Some increases may sound necessary until I read on to an interview with a former insurance regulator.  The interview said that this company is known for making sharp increases to make rates unaffordable to older customers on fixed incomes. The older, sicker customers that have paid consistently for decades drop out of coverage, and the company then offers low rates to the young healthy customers they lost earlier. The purge makes them much more profitable over a short time. It seems, at best, unethical but apparently not illegal. I would be willing to bet no retired executive of that insurance company has to worry about an illness bankrupting them.

In my opinion, health care reform has been stopped. One of the things insurance companies want is for every American to have to buy their products, what they don't want is to have to cover sick people. Does that make sense to anyone?  I am guessing if any law is passed, it will have an out for insurance companies allowing them not to cover really sick people.

Reform has already come to medical offices in the form of the incentive to move to EHRs (electronic health records). If a system meets the criteria for "meaningful use" a large tax break will be available. Electronic claims resolution is another major part of current and future reforms. Health Care Reform in Doubt

I would be the first to say the health care reform bills in the House and Senate are both flawed. I think any bill large enough to cover so many things will have a few items everyone can find fault with. It is a shame though to think many of the common sense portions of the bill may die along with things some don't care for. One example of an item I like is "no denial of coverage because of pre-existing conditions." To the insurance industry it seems unfair. I read an example that one insurance advocate gave. He said, "Allowing coverage with pre-existing conditions is like asking for home owner's insurance as your house burns." I can see that viewpoint, but why then do they take groups every day with members that have pre-existing conditions? The simple answer has to be money. It is a double standard they use to their advantage. We will see what happens but, in my opinion, Congress and both parties are not doing their job if the citizens of this country are not shielded from Insurance Companies' control over the quality of our lives.

The connection is a small stretch but Drew Brees (born in Texas) played at Purdue University in West Lafayette, IN, and he is quarterbacking the Saints into the Superbowl. Peyton Manning was born in New Orleans, LA. and is quarterbacking the Indianapolis, IN Colts to the Superbowl. Small world isn't it?  I wish both teams, and quarterbacks, luck in the big game. I am not a huge football fan, but my son goes to Purdue and I live near Indianapolis, again small world.

I watched the Jets beat a very good Chargers team last week and I was worried when the AFC Championship game started much the same way. It was only in the second half that the Colts made the adjustments necessary and took control of the game. It looked easy and efficient once they had the Jets number. Supreme Court Decision on Political Speech

On Jan. 21st the U.S. Supreme Court decided a case that undid over twenty-five years of campaign finance legislation. The decision, as I understand it, says that organizations such as corporations, unions, and any other special interest group can spend as much money on political advertising as they want. I find this troubling. I feel strongly about individual freedom, and I think everyone should have the right to express their individual opinions.  The problem for me is no one is held accountable for faults, statements or outright lies because it is "political speech."

This ruling however will have a direct effect on individual citizens. One example I heard sited goes like this, a small town mayor and town council reject a large corporation's plans to put a toxic waste dump near their town, before the corp. could legally court the council and mayor hoping to change their minds. Now the corp. can back sympathetic candidates using any kind of "political speech" they can afford. What will happen? The odds are that the town will get new leaders and a toxic waste dump.

Should Everyone Have to Buy Into Health Insurance

Some say it is un-American to make insurance mandatory. Most economists say it's maybe the only way to make coverage affordable without a total government takeover of the medical system in the U.S. I am not smart enough, or all knowing enough, to predict what will work best... I only know this, as long as we do not turn away patients in serious condition from our hospital emergency rooms, taxpayers will continue to pay for the uninsured. I guess the liberal idea is to plan ahead, cut costs and budget for what we pay for anyway. The many young healthy Americans that don't want to pay for insurance and say, "Why should I?", are very quickly the loudest voices at the emergency room desk when they are bleeding from an accidental cut, or severe illness. Since emergency room rates are sometimes ten fold a doctor's visit, we pay a premium for this uninsured person's care.
Cost is the thing. If a medical practice can save money and get quicker re-reimbursements, they are more cost efficient. It pays, literally, to be more cost efficient. EHR, Football

The Indianapolis Colts won their playoff game against Baltimore last Friday night. Since I live in Indianapolis, it was hard to miss the excitement. I am not much of a football fan of late, but it was nice to see my wife and son so happy. The Jets won last weekend too though and the controversial Jets game looms large since the victory over the Colts helped the Jets into the playoffs. What will Colts fans say if the Jets win the AFC title? I don't think I really want to hear that kind of language.

EHR: Just One File

I have been in medical offices as they work many times and I try to stay out of the way while working on computer equipment. I have seen one single lost patient file folder bring an office to its knees. The patient comes to the window to check-in and it begins. The file is not in the stack someone had to pull the day before. The cry goes out, "where is patient X's file?" Everyone stops what they are doing to check the file room, but it is not there so they check again for a miss-filing. The appointment time nears and everyone is running around looking in cabinets and stacks of folders. The patient is in the exam room but the doctor has no file to consult. If they are lucky, someone finds the folder in a stack or on the doctor's desk. If not, I don't know what they do.

The snow we expected is here. I am on the north side of Indianapolis and we have maybe four inches on the ground. The street crews have been out but it is hard to tell. They have a thankless job...They can't win, either they look unprepared or wasteful, I hope the trip home won't be too long.

Health Care Reform Still Alive

I guess the confusing process continues... What will survive the reconciliation process? I hope some of the things that make sense remain when the final bill is voted on, but I am not holding my breath.
