Labor Day Weekend!!!

We are all looking forward to Labor Day Weekend. It gives us a 3-day weekend to be outside and say good bye to Summer! But where did Labor Day come from. It came from one guy named Peter McGuire that thought all workers with a trade should have the right to a 8 hr work day. This would give each worker 8 hrs of work, 8hrs of play, and 8 hrs of sleep to make up their day! Once the movement was passed to protect the workers it was declared Labor Day!!

So when planning your holiday weekend, use your EHR System and Practice Management System to your fullest advantage. Use the E- Prescribing module to approve scripts right from the comfort of your home. If you need to check the patients chart first, no problem simply log in to your EHR System from home and pull up the patient chart.

EHR Systems - Clinical Setup Training

Yesterday I conducted a training session on E-Prescribing, Allergies, E-Encounters, and Tasking. This is the training session that Bradford-Scott refers to as our Clinical Setup Training Session. When purchasing an EHR System from Bradfordscott, we evaluate your need for particular classes and advise you on what classes to take in what order. After completing the EMR training Classes your practice should be able to use the EHR System to document your daily office visits.

This particular EHR training session covered the follow areas with a brief description of each:
  • E-Prescribing- Writing medication prescriptions from the EHR System and sending the script through a secure portal to the pharmacy. Receiving prescription request from the pharmacy through the EHR Software.
  • Allergies- Creating an Allergy list for each patient in which the system will automatically cross reference when sending a medication.
  • E-Encounters- Electronic form that resembles your Superbill, Encounter Form, or Fee Sheet. To allow for a favorites list in the system that applies to your practice.
  • Tasks- The ability to send task to individuals within the office. Also a portal in which the provider can receive and review RX Request and Lab Results.
After the training session I encourage staff members to use their new skills to setup favorite list and settings that meet their office's needs!

EMR Stimulus- Electronic Prescribing

My husband and I own a 109 year old house that we are restoring. Our current project is windows, insulation, and new siding. This is a huge undertaking, but it is actually going fairly well. We bought 16 new windows this spring to do the project. I made sure that they were Energy Star qualified so we would qualify for the $1500 credit. Well...... completing the paperwork for my Tax credit I find that the windows are .001 UVFR away from qualifying for the credit. Lesson learned, make sure you qualify before purchasing!

Point of the story is dealing with the requirements for tax credits can be tricky. So when you are implementing an EMR System and Electronic Prescribing Software, be sure to fully understand Meaningful Use and all requirements for receiving your EMR Stimulus money.

Electronic Health Records- Flowsheets

One of the coolest features in Bradford-Scott's EMR System is the Flowsheets screen. We can customize your EMR system to show the items that you wish to track and also group them together based on condition that you are trying to monitor.

Here are some of the features:
  • Customize list of vitals, findings, and lab results to show on the patients flowchart. in the Patient Records System.
  • Graphing- Display a visual graph of the change in a component of the flowsheet over time. You can graph one item or several items together.
  • Medication Duration- The screen will display the medications that the patient is taking in comparison to the items that you are tracking in your flowsheet. This will quickly show the provider how the medication is affecting the patients findings.
  • Growth Chart- There is a built in growth chart that will plot and child's height and weight automatically.

Flowsheets are just another great feature in Electronic Medical Records that will make it easier for providers to care for their patients.



Continuing EMR Education

This week at Bradford-Scott we have a Technical Trainer in to provide additional training on our EMR System and Medical Software System. There are some things we already know and some things that are new to us. It is important to continuously educate yourself on your systems for there might be something that makes your job easier!

Continuing Education is important to our Electronic Health Records, and Medical Software System clients. Bradford-Scott Data Corp provides free classes that can either be attended in a webinar or classroom environment. These classes cover different areas of the software systems. This allows you to take courses on areas that are most needed.

I have a Medical Software client that attends a basic overall training session once a year. This person leaves the class with something new each time. She says that she will continue to come once a year because she has found so many shortcuts that make her day to day tasks easier.





EMR Report for the Week

This week I had a training session on Tuesday evening from 5-7pm on our Electronic Medical Records Software program. Bradford-Scott offers training times during the day and in the evenings to give your practice time to learn the EMR System and not interfere with your patient load. The training session was covering the Provider Desktop and Patient Charting. These are two areas of our EMR product that will be used daily by the clinical staff and providers. 

The Provider Desktop consists of the providers schedule, Open Task, Open Encounters, and Open Orders. Our EMR System starts the provider out on this screen as a snapshot of all the items they would typically want to monitor.

Patient Charting was the next topic covered. There are many items in this area required to receive Medicare Incentives. Tabs represent different areas of the patients chart. Much like the tabs that are in your paper chart. Except these are items are easy to read and update. There is no need to dig around the chart in search of a document, everything is properly stored under it's specific tab as it is entered in the system.

The training session went well the doctors were more than thrilled with the Health Maintenance Screen. He quoted that to be his favorite screen of the system so far! 

Utilize Your EMR System

So now that you have purchased an Electronic Medical Records System you are having problems adjusting to documenting your patients visit on your new system. You may be feeling like you are so focused on your computer that you are not even looking at your patient. You may feel like it takes you three times as long to do the same amount of patient charting. Here are a few tips to make EMR Adoption an easier process.


  1. Start the visit without your computer to maintain that patient doctor relationship
  2. Face the patient with the laptop or tablet on your lap. This way it is easier to look at the patient from time to time and try to keep that patient relationship strong. 
  3. Practice, Practice, Practice...before going live on the EMR system take a few visits from each day and enter into the system. It may be extra time added to your work day but will make you feel more confident when the patient is in front of you.
  4. Only choose certain visits and start slow. Do not plan on documenting a whole days worth of patients visits your first day, most likely you will get frustrated and quit using it all together.
  5. Do not get hung up on one particular finding that you want to document and cannot find. Write it down and then ask your support team later.

Electronic Medical Records

A few years ago you could mention the acronym EMR and nobody knew what you were talking about. Today you mention EMR and everybody knows what it is, and it does not matter if you are speaking with a medical professional or patient. So what is the point of EMR and how can you benefit from implementing your Electronic Medical Records System?

Electronic Medical Records are used to create better tracking and reporting on patient care to ultimately allow providers to better care for their patients. One of the major features is the Health Maintenance piece. This feature will automatically put the patient in a preventative group of health guidelines based on age and sex. It will also apply a set of care condition guidelines based on the patients problem list.

The doctor is reminded of these care guidelines every time a patent's chart is accessed. As these guidelines are satisfied they will be removed from the list until they are due again. This feature is key to ensure that your patients are being treated for everything that is needed for them individually.

E-RX, E-prescribing, Electronic Prescriptions

Is your practice using an electronic script writing software? If not you might want to invest in one. This is not only a more secure way to submit patient medications to the pharmacy, but there is also a kickback offered to all medical offices that are using a true e-script medical software product from Medicare. This functionality will also be required for your practice to qualify for meaningful use and receive government stimulus money.

Imaging in Electronic Medical Records

Look around your office and notice all the stacks of paper everywhere. With an integrated EMR system all the stacks of paper can be eliminated through document imaging.  Most EMR vendors will discuss converting your paper chart into an electronic chart. However they do not focus on all the other paper documents in the office. Each Medical office receives, radiology reports, discharge letters, referral letters, lab results, and many other items daily in a paper format.

Electronic Patient Charting - Imaging

Many medical offices focus on Practice Management Software to handle their scheduling and billing. They also focus on a Electronic Medical Records software to document their patient visits and health care. However there is another piece of the puzzle and that is Patient Charting or Imaging.

Document Imaging is a great way to get started on a EHR System before fully implementing your EHR System. Document Imaging consist of all the documents in a patient chart. Usually falling into one of these categories:
  • Patient Registration
  • Medication Lists
  • Patient Visits
  • Labs
  • X-Rays
  • Correspondence
After your practice has implemented a EHR system most of these items are generated or electronically received in the system. However until you are using the system the practice will need to rely on the information from their existing paper patient charts. Using a Patient Charting System or Imaging system will allow the practice to get their patient charts in the system to ease the transition to a full blown EHR System.

The implementation for an EHR system is going to determine if you succeed or fail in using an EHR System in your medical practice. The government is forcing all medical offices to move to an Electronic Health Records System. So it is only a matter of time before your office adopts an EHR system.

Planning Phase

A trained EHR implementation specialist will come to your office and discuss your EHR goals and preform a work flow analysis. Next we will discuss the phased implementation plan that we currently use as a base. An in-house champion or super user will be assigned as a main point of contact between your office and our staff. We will then take the information we have gathered and customize a training plan that will fit your office. The training plan is discussed in full and then a road map with dates is created.

Training Phase

OB GYN EMR

There are many features in our Electronic Medical Records Software that I really like and the seem to be very easy for the doctors to use. However there is one feature the EMR System has that seems to get overlooked and that is the OB Chart.

Now I 39 weeks pregnant and about to pop, so maybe I take special interest in this feature for my own personal reasons. But since the OB Medical Practices use standard forms for patient charting, the programmers were able to develop screens that mimic these forms. Electronic Patient Charting for OB visits could not be made easier.



































It is extremely easy to track each patent's visits through their 40 weeks of pregnancy and view them all in this one screen. You can also track all lab results, Ultrasounds, health risk, and tests. It will even provide forms for the doctor to enter the patent's delivery information and postpartum visits that will also display on this main screen.

I know a lot of offices will keep separate charts for OB patient during their term. This system will even allow the doctor do do this. Even though all the information is available to the doctor it will display all the OB information together in a separate area.

What is EMR Adoption?

EMR Adoption is when the medical practice implements a Electronic Medical Records System to handle their patient charting and medical records. Currently in the US, the EHR users are only at 15%, however 23% are in the works of implementing an EHR system in the the next 12 months according to Modern Medicine.

There are many reasons for EMR adoption. It could be to reduce the amount of errors from clinical staff, or to get the funds available from the EHR Stimulus. There are many advantages to the medical service providers, however there are also many benefits to the patient.

An EMR System will code the doctors visits appropriately so that Insurance will cover more of the expense with less left to the patient responsibility. EMR Systems also allow the doctor to easily prove medical necessity so that insurance companies cover more services and equipment. There are also smaller benefits, such as the ease of access to your own medical records and patient flow tracking so the doctor can report on the time you wait in the waiting room. This will allow them to change their workflow and get you in and out.

EHR on the road!

This week I am heading to a Family Practice that is implementing our EHR System in Northern Indiana. This will be the practice's second day with Bradfordscott onsite for support and training.

In the last week the Family Practice Medical Office has been using the EHR system to do the following:
  • Check IN/Out Patients
  • Create a Medication List and Send E-RX
  • Document Patient Allergies
  • Using Tasking for Phone Messages
  • Document Patient Vitals
  • Record Patient Past Patient, Social, and Family History
  • Document Office Visit
The office has done very well for the first week using their new EHR system and are ready to see what else they can learn and use from the system. At Bradfordscott  we determine the most useful tools and train and implement those features first. Once the practice is doing well on those items, we move on to the next set of features.

So far the doctor has been very impressed with how easy the product is to use. She is currently using an EHR System at other locations where she treats patients and has stated how much she likes this system and is excited to see what else they can do.

Correspondence in EHR

Correspondence can refer to any communication between remote parties. This could be email, letters, cards, forums, blogs.... there are a million different types of correspondence. So what does correspondence mean in Electronic Medical Records.

Our Medical software system at Bradfordscott will allow the practice to create correspondence templates pulling data from many areas  starting with the basics, the patients demographic data all the way to the doctors medical findings and plan.

We are able to setup correspondence to pull the information needed for the doctor to effectively communicate with the click of a button.
  • Correspondence for the patient to get Orders completed at a hospital or radiology facility.
  • Correspondence that will thank a patient for coming in and give them a copy of the doctors suggested plan.
  • There is also correspondence that refers a patient to another doctor.
  • Specialty correspondence that pulls in a summary of the patients diagnosis and plan to send back to their referring doctor or medical practice.

Making the small things count in EHR!

There are many features in our EHR system that make documenting a provider-patient visit easier and more efficient. When you are shopping EMR Systems you will hear about meaningful use, e-prescribing, customized encounter notes, and EM coding, Practice Portal, however there are a lot of small features that can make your practice much more efficient that nobody ever looks for.

Here is a list of just of few of the smaller features that you should look for in an EHR system:
 

  • Tasking- the ability to send task to other people and yourself, even record the work on the patients chart.
  • Phone Messaging- no sense in tracking down the doctor to answer a quick question, send him a phone message in the system
  • Profile picture- taking a picture of the patient to put in the EHR system to help the doctor remember what patient he is about to see.
  • Medscape interface- Pulls patient education on their diagnosis which can be easily printed to hand to the client.
  • Flowcharts- there are customizable flowcharts to document lab results with a medication dosage report alongside to show how a certain medication has affected a patients health.
  • Health Maintenance- a list of preventative care items according to the patients age and needs that will generate reminders for the providers and that patients to keep up on their preventative care.
  • Intertrop- the ability to pull in patient charts from a referring doctor electronically and import them into your EHR system

EM Coding in Electronic Medical Records

What is EM Coding? EM coding is the process of assigning a Physician-Patient visit a 5 digit CPT code for Medical Billing purposes. These codes are national, standard codes used by all medical offices and Insurance companies to pay practices for the services provided.
