I recently took my glasses to a local optomatrist that has a chain of locations in our area. I did not buy my glasses there, but I was hoping they could help me fix the lens that had popped out of the frame. The woman at the office was very nice and tried for several minutes to repair the glasses, but was ultimately unable to do so. She kindly offered to send the glasses to their lab at another location, and let me know this would be a complimentary service. They would call me a few days later and I could pick them up. I was so pleased with this store's service, and left with a good feeling about leaving my glasses in their care.

No response from the optomitrist. I decide to give them a few days as I was quite busy that week anyway.

No response from the optomitrist. I called the office, was put on hold, my number was taken and I was assured someone would return my call in a few minutes. I know they are a busy location, so I was not alarmed.

I have not received a call back, and I have not received any information as to why my glasses are missing. A return call to the optomitrist results in my number being taken again and another assurance that someone will call me back in a few minutes. I wait several hours, and then call them again. I am beginning to think that my $600 glasses have been lost! This time I am told that my glasses were shipped to the wrong store, and that they will be shipped to the correct store in a few days. The office will call me when they arrive. I am hoping that this really is the case. Unfortunately, after this poor experience, I am unlikely to ever use this store again.

I never really paid too much attention to customer service before I began working for Bradford Scott Medical. Now I feel like I pay more attention to the service I receive than I do to the product I am purchasing!

Imaging in Electronic Medical Records

Look around your office and notice all the stacks of paper everywhere. Final Rules for EHR Adoption

It's a historic week for EHR adoption!  On Tuesday, July 13, 2010, the final regulations for EHR meaningful use were released.  The original 25 proposed objectives have been broken down into 15 core objectives and 10 menu objectives.  To display meaningful use of EMR adoption and qualify for CMS incentive money, practices must meet all 15 core objectives, and choose from the 10 menu objectives.  (source: www.hitechanswers.net)

To encourage more participation, required benchmarks have been relaxed for stage one compliance.  This means that once you implement electronic health records, you can qualify for incentive money even more quickly.

My husband & I celebrated our 1 year wedding anniversary last Sunday! We have never been happier, and are truly excited to see what life holds for us! Thankfully the last year has had far more ups than downs, but it was great to know that my husband was there for me through it all.

Indianapolis hospitals going after specialists

Indianapolis hospitals are going after specialists like crazy.  Cardiologists and orthopedic surgeons never even considered becoming an employee of the hospital 5 years ago.  Today they are lining up at the door of the hospitals and taking the payouts to assure their financial security is going to be there in the next 2-5 years.  With all the government reductions with Medicare and Medicaid, these specialists continue to take huge pay cuts.  They are scared to death to see what the future holds and are more than willing to what was once thought to never happen, sell out to the hospitals!

Ohio EMR

Tomorrow I'm headed to the Queen City, Cincinnati.  I'm taking the kids to an outdoor rock concert during which it should be a comfortable 95 degrees (with a 103 degree heat index).  I haven't decided whether I'm staying to watch the screaming 'tweens or heading over to watch the thoroughbreds at River Downs.

Speaking of Ohio, it is the state in which we have the highest adoption rate of electronic medical records.  Of our four Intergy by Sage practice management clients, 100% of them are either live with or headed toward EMR adoption!  Our Ohio EMR clients aren't messing around.  In fact, our first Intergy EHR client was from Ohio and they have been using OB GYN EMR for quite some time.

Does Your EHR Make You Money?

Are you actually getting paid for the work that you do?  With the increased popularity of electronic health records, practices are streamlining paperwork, reducing transcription costs, increasing patient volume and more efficiently writing prescriptions.  All of these are crucial to daily work flow and cost control, but is your practice, in fact, increasing it's revenue?

Current studies show that approximately 40% of E&M visits are under-coded by physicians, i.e. the level visit coded is less than the actual level of work performed and documented.  Physicians will often deliberately under-code to avoid audits, thereby leaving thousands of dollars on the table.  This is an obvious disservice to the provider and ultimately only benefits the insurance companies.  EHR can be the solution to this problem.  Top EHR vendors offer an E/M coding solution that is actually built-in to the patient visit, thereby allowing the software to suggest a code after documentation is completed.  There is, in all cases, subjective data that will be entered by the physician, but the completed entry will reflect it as part of the permanent record.

A recent 2-year study was made of a multi-specialty group of 25 providers.  The coding of their newly implemented EHR system reflected the following:
  • Compliance increased from 72% to 94%;
  • revenue increased 12% overall; and,
  • on average, each physician saw 8% less patients during the first year of implementation.

I was reading one of the Star Wars novels last night (yes, sci-fi geek here) and realized that the Force aspect could easily be applied to purchasing and implementing an EMR system.

What is EMR Adoption?

EMR Adoption is when the medical practice implements a Electronic Medical Records System to handle their patient charting and medical records. Currently in the US, the EHR users are only at 15%, however 23% are in the works of implementing an EHR system in the the next 12 months according to Modern Medicine.

There are many reasons for EMR adoption. It could be to reduce the amount of errors from clinical staff, or to get the funds available from the EHR Stimulus. There are many advantages to the medical service providers, however there are also many benefits to the patient.

An EMR System will code the doctors visits appropriately so that Insurance will cover more of the expense with less left to the patient responsibility. EMR Systems also allow the doctor to easily prove medical necessity so that insurance companies cover more services and equipment. There are also smaller benefits, such as the ease of access to your own medical records and patient flow tracking so the doctor can report on the time you wait in the waiting room. This will allow them to change their workflow and get you in and out.

With the benefits to the provider and the patient, why doesn't every medical office have an EMR System? It takes a lot of time and effort on the providers side to learn the product and modify the system to fit their office needs. EHR on the road!

This week I am heading to a Family Practice that is implementing our EHR System in Northern Indiana. This will be the practice's second day with Bradfordscott onsite for support and training.

In the last week the Family Practice Medical Office has been using the EHR system to do the following:
  • Check IN/Out Patients
  • Create a Medication List and Send E-RX
  • Document Patient Allergies
  • Using Tasking for Phone Messages
  • Document Patient Vitals
  • Record Patient Past Patient, Social, and Family History
  • Document Office Visit
The office has done very well for the first week using their new EHR system and are ready to see what else they can learn and use from the system. At Bradfordscott  we determine the most useful tools and train and implement those features first. Once the practice is doing well on those items, we move on to the next set of features.

So far the doctor has been very impressed with how easy the product is to use. She is currently using an EHR System at other locations where she treats patients and has stated how much she likes this system and is excited to see what else they can do.

When shopping an EHR system be sure to ask the Vendor how the product is implemented. It is very important to the success of the implementation that you have a plan that meets your office's needs.

What is Physician Quality Reporting Initiative (PQRI)?

In 2007, PQRI established a financial incentive for Eligible Professionals to participate in a voluntary quality reporting program.   If EPs uses a minimum of 3 PQRI measures for a specified group of patients, that Eligible Professional can earn a bonus payment of 2% of all their Medicare billing for one year.  In 2010 there are 179 PQRI measures and 13 different groups.

Eligible Professionals can use two ways to submit PQRI measures to CMS.

1. Claims Reporting Method: The billing system uses specialized CPTII codes that are submit as part of the normal claim.

2. Registry Reporting Method: EPs can use a CMS approved registry to submit data for collection multiple times throughout the year.
