This week Bradford-Scott Data Corp traveled to Ohio to implement our EHR System at an OBGYN Medical Practice.  The practice has three providers and a supportive clinical staff. Bradford-Scott has done much research on how to successfully implement an EHR System and we find that taking a client live in two phases is usually the best method. This gives them time to absorb the information they are trying to learn and then use it in a live atmosphere.

Is Your EHR Software Going To Be Able To Prove "Meaningful Use"?

There is Government Stimulus Money to be had!  Are you in interested in being a recipient?  If so, you must implement and prove "Meaningful Use" prior to 2014 to get anything.  And, the sooner you participate, the more money you will be eligible for.

However, what is "Meaningful Use"?  Well, the government has put out a proposed list of 25 stage one objectives for meeting Meaningful Use with any EHR System:
  1. Use CPOE
  2. Implement drug-drug, drug-allergy, drug-formulary checks
  3. Maintain an up-to-date problem list of current and active diagnoses based on ICD-9-CM or SNOMED CT
  4. Generate and transmit permissible prescriptions electronically
  5. Maintain an active medication list
  6. Maintain an active allergy list
  7. Record demographics: preferred language, insurance type, gender, race, ethnicity, and date of birth
  8. Record and chart changes in vital signs: height, weight, blood pressure, calculate and display: BMI, and plot and display growth charts for children 2-20 years, including BMI
  9. Record smoking status for patients 13 years old or over
  10.  Incorporate clinical lab-test results into EHR as structured data
  11.   Generate lists of patients by specific conditions to use for quality improvement, reduction of disparities and outreach
  12.   Report ambulatory quality measures to CMS or the States
  13.   Send reminders to patients per the patient's preference for preventive/follow up care
  14.   Implement five clinical decision support rules relevant to specialty or high clinical priority, including diagnostic test ordering, along with the ability to track compliances with those rules
  15.   Check insurance eligibility electronically from public and private payers
  16.   Submit claims electronically to public and private payers
  17.   Provide patients with an electronic copy of their health information (including diagnostic test results, problem list, medication lists, allergies), upon request
  18.   Provide patients with timely electronic access to their health information (including lab results, problem list, medication lists, allergies) within 96 hours of the information being available to the EP
  19.   Provide clinical summaries for patients for each office visit
  20.   Capability to exchange key clinical information (for example, problem list, medication list, allergies, diagnostic test results), among providers of care and patient authorized entities electronically
  21.   Perform medication reconciliation at relevant encounters and each transition of care
  22.   Provide summary care record for each transition of care and referral
  23.   Capability to submit electronic data to immunization registries and actual submission where required and accepted
  24.   Capability to provide electronic syndromic surveillance data to public health agencies and actual transmission according to applicable law and practice
  25.   Protect electronic health information created or maintained by the certified EHR technology through the implementation of appropriate technical capabilities

What type of system do you suppose the Doctors and scientist working at the Smithsonian Institute would prefer? As an avid viewer of the Bones series I know they use the latest and greatest research software and techniques available. They can reconstruct a complete skeleton with just a few bones and some x-rays. Weekly, we see them come up with the face of the victim from just the skull or even pieces of the skull. They do amazing work each week. Yes, it is T.V. and viewers expect them to be able to do the unexpected. But I am sure that Temperance Brennan "Bones" would only settle for the most relevant EHR software. Not only would the Smithsonian Institute want to use the best equipment they could afford but since the lab works with the FBI consistently, the government would also want them to use contemporary software. Bones would certainly want to check out Intergy by Sage before making an educated discussion about which EHR software to choose. I am sure that she would want to act now while there is Stimulus money available, instead of waiting until she was mandated to purchase an EHR system. She would want to be able to use government funds and buy now instead of waiting until later and paying more. Internal Medicine Practice Saves Over $850 Monthly with Intergy EHR

8 Grant Application Tips

I found this on hhs.gov and thought that it may be helpful to some of you. EHRs (Electronic Health Records) are here and isn't going anywhere.  Now is the time to get serious because it isn't a question of why anymore, it is the reality of when.   Call Bradford-Scott today and we'll walk you through it all to ensure that you receive the maximum amount of incentives available to you!
  1. Include a DUNS Number. A DUNS Number must be included in order for an application to be reviewed.  DUNS numbers can be obtained by accessing www.dunandbradstreet.com or by calling 1-866-705-5711. Include the DUNS number next to the OMB Approval Number, which is located in the upper right corner of the application face page.
  2. Keep the audience in mind. Reviewers will use only the information contained in the application to assess the application. Therefore, the applicant should be sure the application and responses to the program requirements and expectations are complete and clearly written. Do not assume that reviewers are familiar with the applicant organization. Keep the review criteria in mind when writing the application. 
  3. Start preparing the application early. If applying electronically through Grants.gov please ensure that adequate time is allotted to register and download applicable software and forms. Grants.gov offered a "Webcast" (registration required) entitled "Get Started with Grants.gov" that provides start-up requirements and tips.
  4. Follow the instructions and application guidance carefully. The instructions call for a particular organization of the materials, and reviewers are accustomed to finding information in specific places. Present information according to the prescribed format.
  5. Be brief, concise, and clear. Make each point understandable. Provide accurate and honest information, including candid accounts of problems and realistic plans to address them. If any required information or data is omitted, explain why. Make sure the information provided in each table, chart, attachment, etc., is consistent with the proposal narrative and information in other tables. 
  6. Be organized and logical. Many applications fail because the reviewers cannot follow the thought process of the applicant or because parts of the application do not fit together. 
  7. Be careful in the use of appendices. Do not use the appendices for information that is required in the body of the application. Be sure to cross-reference all tables and attachments located in the appendices to the appropriate text in the application. 
  8. Carefully proofread the application. Misspellings and grammatical errors will impede reviewers in understanding the application. Be sure pages are numbered (including appendices) and that page limits are followed. Limit the use of abbreviations and acronyms, and define each one at its first use and periodically throughout application.

8. How will the data be securely stored to protect your patients’ privacy?
9. How often will the software/hardware need to be upgraded and what will this cost?
10. Is the solution scalable as your practice grows?

By now, you have a couple of vendors who are standing out from the rest. The functionality meets the needs of your practice and you are starting to get excited about how the new practice management software and EHR solution can help your practice. Now it’s time to get references and ask your peers what they think about the software. Here are some questions you can ask:

1. How smooth was the implementation?
2. How well was training conducted?
3. What do you like best about the software?
4. What do you like least about the software?

Hopefully this has helped to further narrow your selection. Remember that you get what you pay for and you shouldn’t waste your time on products that don’t meet the needs you identified at the beginning of the process. EMR - why aren't you using it yet?

I have to say that on the whole, we really are a funny race - simply because we are so contradictory.  Take a look around and you will see someone with an ipod, cell phone, playing video games, watching TV or using a computer - after all, it is the age of technology.  Right?

Hmm,  look at the health industry.  When it is so vital that the patients' information about their condition, medicines, etc be accurate and available, we are bound by columns of paper charts(if you can find them),  sticky notes for patient messages,  illegible notes and prescriptions. 

What is everyone waiting for?  Here are some of the reasons I've heard:
  • No time
  • Costs too much
  • Heard too many stories of failed implementations
Stop and take a look at the facts.

Studies have shown that using an electronic medical records system, EMR, will increase your efficiency, decrease office costs and improve patient care.

Still concerned about cost?  The American Recovery & Reinvestment Act (ARRA) offers an incentive (up to $44,000 over 5 years) for physicians to implement an approved electronic medical record system.
