Electronic Health Records- Flowsheets

Friday, August 27, 2010 by Carey Fulwider

One of the coolest features in Bradford-Scott's EMR System is the Flowsheets screen. We can customize your EMR system to show the items that you wish to track and also group them together based on condition that you are trying to monitor.

Here are some of the features:
  • Customize list of vitals, findings, and lab results to show on the patients flowchart. in the Patient Records System.
  • Graphing- Display a visual graph of the change in a component of the flowsheet over time. You can graph one item or several items together.
  • Medication Duration- The screen will display the medications that the patient is taking in comparison to the items that you are tracking in your flowsheet. This will quickly show the provider how the medication is affecting the patients findings.
  • Growth Chart- There is a built in growth chart that will plot and child's height and weight automatically.

Flowsheets are just another great feature in Electronic Medical Records that will make it easier for providers to care for their patients.

EMR Report for the Week

Friday, August 20, 2010 by Carey Fulwider
This week I had a training session on Tuesday evening from 5-7pm on our Electronic Medical Records Software program. Bradford-Scott offers training times during the day and in the evenings to give your practice time to learn the EMR System and not interfere with your patient load. The training session was covering the Provider Desktop and Patient Charting. These are two areas of our EMR product that will be used daily by the clinical staff and providers.

The Provider Desktop consists of the providers schedule, Open Task, Open Encounters, and Open Orders. Our EMR System starts the provider out on this screen as a snapshot of all the items they would typically want to monitor.

Patient Charting was the next topic covered. There are many items in this area required to receive Medicare Incentives. Tabs represent different areas of the patients chart. Much like the tabs that are in your paper chart. Except these are items are easy to read and update. There is no need to dig around the chart in search of a document, everything is properly stored under it's specific tab as it is entered in the system.

The training session went well the doctors were more than thrilled with the Health Maintenance Screen. He quoted that to be his favorite screen of the system so far!

Utilize Your EMR System

Friday, August 20, 2010 by Carey Fulwider

So now that you have purchased an Electronic Medical Records System you are having problems adjusting to documenting your patients visit on your new system. You may be feeling like you are so focused on your computer that you are not even looking at your patient. You may feel like it takes you three times as long to do the same amount of patient charting. Here are a few tips to make EMR Adoption an easier process.

  1. Start the visit without your computer to maintain that patient doctor relationship
  2. Face the patient with the laptop or tablet on your lap. This way it is easier to look at the patient from time to time and try to keep that patient relationship strong.
  3. Practice, Practice, Practice...before going live on the EMR system take a few visits from each day and enter into the system. It may be extra time added to your work day but will make you feel more confident when the patient is in front of you.
  4. Only choose certain visits and start slow. Do not plan on documenting a whole days worth of patients visits your first day, most likely you will get frustrated and quit using it all together.
  5. Do not get hung up on one particular finding that you want to document and cannot find. Write it down and then ask your support team later.
These are just a few of our tips that we offer when implementing an EMR System. Bradford-Scott has been in the business of Medical Software for over 25 years. With our experience we offer the best support in the business.

Electronic Medical Records

Friday, August 20, 2010 by Carey Fulwider

A few years ago you could mention the acronym EMR and nobody knew what you were talking about. Today you mention EMR and everybody knows what it is, and it does not matter if you are speaking with a medical professional or patient. So what is the point of EMR and how can you benefit from implementing your Electronic Medical Records System?

Electronic Medical Records are used to create better tracking and reporting on patient care to ultimately allow providers to better care for their patients. One of the major features is the Health Maintenance piece. This feature will automatically put the patient in a preventative group of health guidelines based on age and sex. It will also apply a set of care condition guidelines based on the patients problem list.

The doctor is reminded of these care guidelines every time a patent's chart is accessed. As these guidelines are satisfied they will be removed from the list until they are due again. This feature is key to ensure that your patients are being treated for everything that is needed for them individually.

There are many other features of Electronic Medical Records that provide better care to patients. Health Maintenance is just one many. Contact Bradford-Scott Data Corp for more information.

5 Reasons for EHR Now!

Sunday, August 1, 2010 by Jessica Andresen
Why consider an Electronic Medical Records System?

• Improved Coding - With better documentation tools, your coding will be more accurate.  Where there is more accuracy, better patient care and, in most cases,  higher reimbursements will follow.

• Storage - Where are you putting those charts?  Keeping them on-site takes up too much space and keeping them off-site takes up too much of your money.  Use your space for another exam room and use your money for whatever you want, not renting the unnecessary.

• e-Prescribing - Did you know that, on average, your staff spends 10 minutes handling prescription refill calls alone?  How many of these calls are they taking each day?  You're paying them how much per hour?

• Malpractice Premiums - Did you know that some insurance carriers offer discounts to those providers using EHR?  It's true...  Some even offer discounts all the way up to 20%, and for you specialty practices out there, this type of saving would more than cover the cost of your EHR.

• No More Chart Pulls - With an average cost of $5 per chart pull, how much money would your practice save?  The average is 30 chart pulls per day, what would you do with an extra $39,150  per year?

If you wait until you are forced to implement an Electronic Health Records system, will you end up with one that you have no control over? Call us today to view a demo of Intergy EHR.  Intergy EHR meets all of the Stage 1 criteria to receive your incentives.

It's not a question of "if", it's a question of "when".

Imaging in Electronic Medical Records

Friday, July 16, 2010 by Carey Fulwider

Look around your office and notice all the stacks of paper everywhere. With an integrated EMR system all the stacks of paper can be eliminated through document imaging.  Most EMR vendors will discuss converting your paper chart into an electronic chart. However they do not focus on all the other paper documents in the office. Each Medical office receives, radiology reports, discharge letters, referral letters, lab results, and many other items daily in a paper format.

Bradford-Scott Data Corp has an EMR product that can convert all your paper into electronic files no matter how they get to your office. There are many items that come into the office through fax. With a faxing solution these items can be directly imported into the system and sent to the doctor for review and stored in the patient chart.

Another way to cut out the paper is to receive your Radiology reports,  Ultrasounds, hospital reports, and an other paper documentation on an cd and then import them into the system using the imaging feature in your EMR system.

Your Medical Office can even go a step further and scan in all their EOB's into an electronic file cabinet. So can staff information, patient education, and doctors registration. Security settings within the EMR system can be set to only all certain people to see certain types of documents.

The opportunities to eliminate paper in your office are endless. You will not only make it easier to find the medical records you are looking for, but also help environment by cutting back on the use of paper. Electronic Medical Records Systems make it easy to organize your office.

Should we scan our old charts?

Friday, May 21, 2010 by Mike Bessignano

I meet with physician offices on a weekly basis to discuss electronic health records (EHR).  One of the first questions that comes is "should we scan in all our charts?".  There are a couple questions you need to ask yourself to determine whether or not it makes sense to scan your old charts.

1.  Are we paying a fee to store old charts?

2.  Are we able utilize or need the space where the charts reside currently?

If your answers are no to both questions, the action is simple, you don't need to scan in your old charts.  What you will do is scan in active patients charts for patients that are scheduled, staying at least two weeks ahead.  Eventually you will have scanned over time, the active patient population for your practice and would not have burdened yourself with time and money to scan inactive patient data.

If your answer are yes to either of the two questions, then you need to seriously consider scanning those charts and eliminating the costs to maintain those active and inactive charts.  Probably the most cost effective solution for scanning in large amounts of images is bringing in an outside source who can scan large quantities of charts in short amounts of time.  Their cost per page is drastically reduced compared to what it would cost your office to perform this task internally.  This would enable you to clear the space or eliminate the storage cost immediately.

If you are interested in learning more about your options on scanning old charts, contact us at 317-713-2065 or look us up @ http://www.bradfordscott.com/medical.php

Time to do spring clealing - replace your charts with EMR!

Friday, May 21, 2010 by Kathy Reinhardt
It's cleaning day here at Bradford-Scott Data Corporation.    Every spring we take a day to make sure everything sparkles, re-organize and get rid of accumulated clutter.

Have you thought about making a clean sweep in your office?

How much room do your paper charts take that could be used for more productive space?

What about those charts that are no longer active but need to be kept for at least seven years?

It's time to make a change for the better and move to an electronic medical record software system(EMR).  Not only will you find that you can store patient charts electronically, but you can also scan EOB's and other material taking up space in all those file drawers.  With our SAGE Intergy product, you can set security levels for access, so confidential information is protected.

Changing to an EMR system will also provide benefits you didn't"t expect -

Increased efficiency in your office - with instant access to charts, electronic prescriptions and lab orders as well as results, just to name a few.  Once you have implemented an EMR system, you will also notice that patient care has improved as well.

An added bonus to implementing an EMR system now, is the ARRA Regulations that provide Medicare or Medicaid incentive money for implementing an EMR and meeting the meaningful use criteria.  Early EMR adoption can also increase the amount of incentive available per physician.

Are you ready to make a change, but not sure where to start?

Let our Bradford-Scott Data Corporation  medical sales consultants discuss the ARRA Rules and the SAGE Intergy EMR product with you and start you on your way.

Lab Portal Services with EHR

Wednesday, May 5, 2010 by Kathy Reinhardt
How much time do you spend in your office handling lab information?  Are you always scrambling to check on the results - pulling the chart and searching through it for the correct test?  Do you have to go to multiple websites to get the information?

Something to consider when you are purchasing your electronic medical record system is whether or not the software has an interface with laboratories.  One of the recommended requirements for Phase I of the ARRA Meaningful Use Rule requires you to incorporate clinical lab-test results into structured data into EHR.   Entering the results manually would be a huge burden on the practice.

Lab Portal Services is one of many great features of Sage Intergy EHR.   This allows you to
  • create and submit lab orders electronically
  • print the requisition
  • receive the results electronically
  • system automatically post results to the patient chart and sends a task for the doctor to review them.
  • view test results in a flowsheet or graph format
All this information is available at your fingertips.

If you are interested in using Lab Portal Services with SAGE Intergy EHR, contact our medical consultant team at Bradford-Scott Data Corporation

Electronic Patient Charting - Imaging

Friday, April 23, 2010 by Carey Fulwider
Many medical offices focus on Practice Management Software to handle their scheduling and billing. They also focus on a Electronic Medical Records software to document their patient visits and health care. However there is another piece of the puzzle and that is Patient Charting or Imaging.

Document Imaging is a great way to get started on a EHR System before fully implementing your EHR System. Document Imaging consist of all the documents in a patient chart. Usually falling into one of these categories:
  • Patient Registration
  • Medication Lists
  • Patient Visits
  • Labs
  • X-Rays
  • Correspondence
After your practice has implemented a EHR system most of these items are generated or electronically received in the system. However until you are using the system the practice will need to rely on the information from their existing paper patient charts. Using a Patient Charting System or Imaging system will allow the practice to get their patient charts in the system to ease the transition to a full blown EHR System.

There are many benefits to this. It will allow the practice to work out a system that goes with their work flow of getting old and new documents scanned into the patient chart. It will also allow all the staff in the office to find the chart at their finger tips from their desk whenever they need it. Not to mention the doctor will have the chart to view before seeing patients. There is no more searching for patient charts. They are all ready and available.

Bradfordscott Data Corp has a three phase implementation process that will allow medical offices to go live on the Practice Management, Imaging, and Full blown EHR System in phases. This is a very important piece that often gets overlooked. Use it as a stair step in converting your practice to Electronic Medical Records.

6 Reasons why Electronic Health Records

Monday, April 12, 2010 by Mike Bessignano

These days most physicians are being lured to electronic health records (EHR) with Federal Stimulus money.  What they should really be lured by are the true beneficial reasons for adopting EHR.

1.  Better Access:  Never look for a chart again, information at your finger tips, access the chart remotely, etc...

2.  Better Documentation:  Legible documents, updating patient meds and problems at each visit, patients able to update medical history from home and have information downloaded into your EHR, import discrete data from labs, hospitals, etc...

3.  Better Care Management:  Ability to track patient health maintenance with alerts to remind clinical staff of overdue labs, x rays, etc...

4. Better Prescribing:  Less time spent speaking with pharmacists, electronically send scripts to pharmacy, refill scripts in seconds, catch important drug interactions and ability to pull a group of patients for a recalled drug.

5.  Greater Efficiency:  Electronic tasking, batch form letters, eliminate time copy charts, reduce transcription, eliminate chart costs, etc...

6. Higher Income:  Qualify for pay for performance (PFP) bonuses, capture all your charges at time of service, re assign your transcriptionist and filing clerk to help with collections, confidently code at higher levels, etc...

These are 6 good reasons why physicians should be adopting EHR!  To learn more about how an EHR can increase your revenues, reduce your costs and become more efficient, contact Bradford Scott Data Corporation today!

OB GYN EMR

Friday, April 9, 2010 by Carey Fulwider

There are many features in our Electronic Medical Records Software that I really like and the seem to be very easy for the doctors to use. However there is one feature the EMR System has that seems to get overlooked and that is the OB Chart.

Now I 39 weeks pregnant and about to pop, so maybe I take special interest in this feature for my own personal reasons. But since the OB Medical Practices use standard forms for patient charting, the programmers were able to develop screens that mimic these forms. Electronic Patient Charting for OB visits could not be made easier.

It is extremely easy to track each patent's visits through their 40 weeks of pregnancy and view them all in this one screen. You can also track all lab results, Ultrasounds, health risk, and tests. It will even provide forms for the doctor to enter the patent's delivery information and postpartum visits that will also display on this main screen.

I know a lot of offices will keep separate charts for OB patient during their term. This system will even allow the doctor do do this. Even though all the information is available to the doctor it will display all the OB information together in a separate area.

This is just one of the EMR System that Bradfordscott has to offer.

What is EMR Adoption?

Thursday, April 8, 2010 by Carey Fulwider
EMR Adoption is when the medical practice implements a Electronic Medical Records System to handle their patient charting and medical records. Currently in the US, the EHR users are only at 15%, however 23% are in the works of implementing an EHR system in the the next 12 months according to Modern Medicine.

There are many reasons for EMR adoption. It could be to reduce the amount of errors from clinical staff, or to get the funds available from the EHR Stimulus. There are many advantages to the medical service providers, however there are also many benefits to the patient.

An EMR System will code the doctors visits appropriately so that Insurance will cover more of the expense with less left to the patient responsibility. EMR Systems also allow the doctor to easily prove medical necessity so that insurance companies cover more services and equipment. There are also smaller benefits, such as the ease of access to your own medical records and patient flow tracking so the doctor can report on the time you wait in the waiting room. This will allow them to change their workflow and get you in and out.

With the benefits to the provider and the patient, why doesn't every medical office have an EMR System? It takes a lot of time and effort on the providers side to learn the product and modify the system to fit their office needs. This is why it is so important for medical offices to choose a EMR vendor who is experienced in EMR Adoption. A Vendor like Bradfordscott, who has successfully implemented EMR systems. Medical Practices will also want a vendor that offers them unlimited product support.

Bradfordscott develops a training plan specific to each practice to help their EMR adoption to be a smooth as possible. With our help you can enjoy the benefits and so can your patients.

EMR - How do you measure up?

Friday, April 2, 2010 by Kathy Reinhardt
When I learned to audit charts, the phrase they always drummed into your head was -

"If it isn't documented, they didn't do it"

In other words, you won't get paid for it.  That can take your reimbursement down from a level 3 visit to a level 1 - ouch!!

How do you measure up?  Many physicians do the work, but fail to document.  In addition to being vulnerable to an audit, this also hurts you financially as you tend to under code.

When you purchase an electronic medical records system, the changes for how you do your medical charting can be overwhelming.  Things you may automatically check for, but don't always take the time to document are staring you in the face. It does take time to change your habits, but the benefits far outweigh the effort to change.

Why should you care about the benefits?
  • Improved and accurate documentation of patient care - everyone being able to read the prescription or your encounter note decreases errors.
  • Messages for patient care (ex. rx refills, test results)- again documented in the system, not on sticky notes that get lost.
  • Patient chart with information available at a click of a button to multiple users, not left in the trunk of the car.
  • With our SAGE Intergy EHR software, the Medcin database calculates the level of the office visit based on the findings you selected or cited during the visit.
What it comes down to is improving care, staff efficiency and increasing revenue.  Why would you want to wait to make these changes?

Still unsure?  Consider that the EHR Stimulus  for physicians' to qualify for grant money for implementing EHR and meeting "meaningful use" criteria is available over a five year period.  That's the icing on the cake!

Talk to one of our medical consultants at Bradford-Scott Data Corporation today and find out how you can make the change!

Correspondence in EHR

Friday, March 19, 2010 by Carey Fulwider

Correspondence can refer to any communication between remote parties. This could be email, letters, cards, forums, blogs.... there are a million different types of correspondence. So what does correspondence mean in Electronic Medical Records.

Our Medical software system at Bradfordscott will allow the practice to create correspondence templates pulling data from many areas  starting with the basics, the patients demographic data all the way to the doctors medical findings and plan.

We are able to setup correspondence to pull the information needed for the doctor to effectively communicate with the click of a button.
  • Correspondence for the patient to get Orders completed at a hospital or radiology facility.
  • Correspondence that will thank a patient for coming in and give them a copy of the doctors suggested plan.
  • There is also correspondence that refers a patient to another doctor.
  • Specialty correspondence that pulls in a summary of the patients diagnosis and plan to send back to their referring doctor or medical practice.
There are many ways to pull the information recorded in your EHR system into a form that can be emailed, printed, or even faxed to receiving party at a click of a button. Not only does the system pull the information for the provider, but it also stores a copy in the patient chart for reference.

Making the small things count in EHR!

Friday, March 19, 2010 by Carey Fulwider

There are many features in our EHR system that make documenting a provider-patient visit easier and more efficient. When you are shopping EMR Systems you will hear about meaningful use, e-prescribing, customized encounter notes, and EM coding, Practice Portal, however there are a lot of small features that can make your practice much more efficient that nobody ever looks for.

Here is a list of just of few of the smaller features that you should look for in an EHR system:

  • Tasking- the ability to send task to other people and yourself, even record the work on the patients chart.
  • Phone Messaging- no sense in tracking down the doctor to answer a quick question, send him a phone message in the system
  • Profile picture- taking a picture of the patient to put in the EHR system to help the doctor remember what patient he is about to see.
  • Medscape interface- Pulls patient education on their diagnosis which can be easily printed to hand to the client.
  • Flowcharts- there are customizable flowcharts to document lab results with a medication dosage report alongside to show how a certain medication has affected a patients health.
  • Health Maintenance- a list of preventative care items according to the patients age and needs that will generate reminders for the providers and that patients to keep up on their preventative care.
  • Intertrop- the ability to pull in patient charts from a referring doctor electronically and import them into your EHR system
These are just a few of the smaller features that are not highlighted when shopping for EHR but can make all the difference. Bradfordscott specializes in training you on all the features that available to make your day easier!
