PQRI Measures

The new buzz-phrase in the health care industry is PQRI Measures.  What exactly does that mean?  PQRI stands for Physician Quality Reporting Initiative.  It sounds like just another industry committee but if you dig deeper it can mean money.


By using your physician billing software along with your electronic medical records software, your practice can qualify for Medicare incentives.  Simply report your PQRI measures on a CMS claim form, to a qualified PQRI registry, or through a qualified
electronic health records (EHR) product.

Updated rule on referring physicians on your claims

CEDI Ordering/Referring Provider Edits 

The Centers for Medicare & Medicaid Services (CMS) has updated the “OrderingReferringReport” file containing the National Provider Identifier (NPI) and the name (last name, first name) of all physicians and nonphysician practitioners who are of a type/specialty that is eligible to order and refer in the Medicare Program and who have current enrollment records in Medicare (i.e., they have enrollment records in Provider Enrollment Chain & Ownership System (PECOS) that contain an NPI).  

This file is downloadable from the Medicare provider/supplier enrollment Web site www.cms.hhs.gov/MedicareProviderSupEnroll, click on “OrderingReferringReport” on the left-hand side.

Note: The “Ordering Referring Report” is a large .pdf file (20000 KB). Due to the large size, CEDI suggests you right click and select “Save as” before attempting to open this file. 

The Common Electronic Data Interchange (CEDI) receives PECOS updates Monday through Saturday and some holidays with physicians and nonphysician practitioners who are of a type/specialty that is eligible to order and refer in the Medicare program. These updates are incorporated by CEDI to be used to perform the PECOS edits. 

In order to determine if the ordering and/or referring provider submitted on the claim to CEDI matches the records with PECOS, CEDI performs the following steps: 

1.      CEDI first checks if the Ordering/Referring Provider's NPI submitted on the claim is on the file CEDI has received from PECOS.

  • If not, the CEDI edit will set.

2.      If the NPI matches the PECOS file, CEDI then verifies the Ordering/Referring Provider's name as submitted on the claim matches the name associated with the NPI.

  • CEDI compares the first four characters of the last name to the Ordering/Referring Provider's name received from PECOS
  • CEDI compares the first letter of the first name to the Ordering/Referring Provider's name received from PECOS
    • If the first or last name does not match, the CEDI edit will set.

Suppliers should:

  • Verify with the provider who ordered or referred services that they are enrolled in PECOS.
  • Submit the Ordering/Referring Provider's name as enrolled with PECOS.
    • Do include spaces in last names. For example, if the ordering/referring provider’s last name is “A BCDE” do not submit the last name as “ABCDE”
    • Do include special characters in last names. For example, if the ordering/referring provider’s last name is “A-BCDE” or “A’BCDE” do not submit the last name as “ABCDE”
    • Do not use nicknames (“BOB” for “ROBERT”)
    • Do not use credentials (“DR JOHN” for “JOHN”)
  • Submit the name in upper case letters. Lower case letters will not find a match on the PECOS file as the PECOS file only contains upper case.
  • Submit the Type 1 (individual physician’s) NPI and name of the Ordering/Referring Provider. If the Type II (physician’s group) NPI and name is submitted, a match will not be found on the PECOS file. 

When the CEDI front end edits begin rejecting claims, suppliers will be able to correct and resubmit the claims once the physician or nonphysician practitioner has updated their enrollment in PECOS. The PECOS edits are based on date of receipt and not date of service.

***It is tough enough to keep up with all the changes that billers need to know to get their claims paid. Changing Times

This week we all sprung ahead to Daylight Savings Time.  Has your body figured it out yet?  Surprisingly I've been getting up earlier this week than normal.  I guess that small of a change didn't affect me so much.

There are many more things going on in these changing times of health care.  You're got proposed health care reform, PQRI measures being implemented, and of course the ARRA Program encouraging providers to consider EHR adoption.

Since I've been in the medical software business there have been many changes.  When I began, there were blue HCFA forms and bisync electronic claims modems.  Now there are CMS Incentive programs for EMR adoption, electronic prescriptions, and Internet electronic claims.

Is Your EHR Software Going To Be Able To Prove "Meaningful Use"?

There is Government Stimulus Money to be had!  Are you in interested in being a recipient?  If so, you must implement and prove "Meaningful Use" prior to 2014 to get anything.  And, the sooner you participate, the more money you will be eligible for.

However, what is "Meaningful Use"?  Well, the government has put out a proposed list of 25 stage one objectives for meeting Meaningful Use with any EHR System:
  1. Use CPOE
  2. Implement drug-drug, drug-allergy, drug-formulary checks
  3. Maintain an up-to-date problem list of current and active diagnoses based on ICD-9-CM or SNOMED CT
  4. Generate and transmit permissible prescriptions electronically
  5. Maintain an active medication list
  6. Maintain an active allergy list
  7. Record demographics: preferred language, insurance type, gender, race, ethnicity, and date of birth
  8. Record and chart changes in vital signs: height, weight, blood pressure, calculate and display: BMI, and plot and display growth charts for children 2-20 years, including BMI
  9. Record smoking status for patients 13 years old or over
  10.  Incorporate clinical lab-test results into EHR as structured data
  11.   Generate lists of patients by specific conditions to use for quality improvement, reduction of disparities and outreach
  12.   Report ambulatory quality measures to CMS or the States
  13.   Send reminders to patients per the patient's preference for preventive/follow up care
  14.   Implement five clinical decision support rules relevant to specialty or high clinical priority, including diagnostic test ordering, along with the ability to track compliances with those rules
  15.   Check insurance eligibility electronically from public and private payers
  16.   Submit claims electronically to public and private payers
  17.   Provide patients with an electronic copy of their health information (including diagnostic test results, problem list, medication lists, allergies), upon request
  18.   Provide patients with timely electronic access to their health information (including lab results, problem list, medication lists, allergies) within 96 hours of the information being available to the EP
  19.   Provide clinical summaries for patients for each office visit
  20.   Capability to exchange key clinical information (for example, problem list, medication list, allergies, diagnostic test results), among providers of care and patient authorized entities electronically
  21.   Perform medication reconciliation at relevant encounters and each transition of care
  22.   Provide summary care record for each transition of care and referral
  23.   Capability to submit electronic data to immunization registries and actual submission where required and accepted
  24.   Capability to provide electronic syndromic surveillance data to public health agencies and actual transmission according to applicable law and practice
  25.   Protect electronic health information created or maintained by the certified EHR technology through the implementation of appropriate technical capabilities

Gold medal

I've been trying to keep up with the Olympics as I have time.  A DVR is a wonderful tool for doing that, although I'm usually a day behind and sometimes know the results before I get to watch.  So far the USA is doing pretty well at winning gold medals.

Today I had a crazy vision:  What if Bradford-Scott Data Corporation was in the physician medical billing or electronic medical records Olympics?  I think we'd be standing up there receiving our gold medals in many events like:

Teamwork

Purdue won at Ohio State last night!  It took some definite teamwork to pull this one off.  On a night when Robbie Hummel was invisible, Grant and Johnson stepped up to help the Boilers beat the Bucks.

Who is Sage?

Caved

For some time one of our system administrators at Bradford-Scott has been exploring caves.  Brad Barcom has become an expert caver in an short amount of time.  Last weekend he invited a few of us from work to go caving at Buckner Cave in Bloomington, IN.  After acquiring permission and signing the release forms, we were off.

I took my 11-year-old Sam for what was promised to be a 2-3 hour trip.  We crawled and crawled, sometimes through mud or water.  Six hours later we were out!  I think we were crawling for nearly four hours of the six!  We saw lots of hibernating bats, a few formations, and a waterfall.  Actually we crawled through the flowing waterfall which was the most difficult but also the most spectacular part of the cave.  It was an experience I'll never forget.



But it's also nice to be back at Bradford-Scott Data Corporation in a comfy, warm, dry chair.  The world of dust, mud, clay and water has change to a world of EMR systems, medical claims software and data conversions.  Though every muscle and joint in my body still aches, I can still use Intergy by Sage and Intergy EHR by Sage from the comfort of this keyboard!  So if you're so inclined, be adventurous!  And if you're a physician or work in physician medical billing, contact us for more information about Intergy by Sage or Intergy EHR by Sage.

Plus, there are more than 400 vendors of EHR/EMR products on the market today. It’s important to exercise caution and perform a thorough due diligence, but where should you start?

By now, you have a couple of vendors who are standing out from the rest. The functionality meets the needs of your practice and you are starting to get excited about how the new practice management software and EHR solution can help your practice. Now it’s time to get references and ask your peers what they think about the software. Here are some questions you can ask:

1. How smooth was the implementation?
2. How well was training conducted?
3. What do you like best about the software?
4. What do you like least about the software?

Hopefully this has helped to further narrow your selection. Remember that you get what you pay for and you shouldn’t waste your time on products that don’t meet the needs you identified at the beginning of the process. Focusing on cost first could rule out some of the best products and minimize your chance to find the best solution that maximizes your potential gains – giving you a shoe that just doesn’t fit.

The other health care reform

What was supposed to be the big health care reform has hit the skids.  Now that the Democrats' super-majority is gone, it looks like it may be even longer before any reform is passed.  When it comes down to it, most people (excluding insurance companies) want some kind of reform.  Physicians don't want their pay cut, patients don't want their benefits cut, but something needs to be done.  My guess is that it's going to be a very slow process.

I looked the AMA's website and it details the following about reform:  The American Medical Association (AMA) has identified seven critical elements necessary to improve access to affordable, quality care and reduce unnecessary costs in the current system:
    • Health insurance coverage for all Americans
    • Insurance market reforms that expand choice of affordable coverage and eliminate denials for pre-existing conditions
    • Assurance that health care decisions will remain in the hands of patients and their physicians, not insurance companies or government officials
    • Investments and incentives for quality improvement, prevention and wellness initiatives
    • Repeal of the Medicare physician payment formula that would trigger steep cuts and threaten seniors' access to care
    • Implementation of medical liability reforms to reduce the cost of defensive medicine
    • Streamlining and standardizing of insurance claims processing requirements to eliminate unnecessary costs and administrative burdens

Medical practice managment tips

Medicare Claims Not Crossing Over to the Supplemental Insurer

National Government Services is informing all providers, physicians, and suppliers to an issue that occurred starting on or about January 1, 2010, and would have negatively impacted their patients' crossover claims.

National Government Services has learned that due to a system issue with the Common Working File (CWF), some claims are not crossing over to the supplemental insurance carrier for benefits.

CMS states that they are working on this issue and they further recommend that providers do the following; 
 

*         Examine your Electronic Remittance Advice (ERA) or standard paper remittance advice to determine if your patients' claims are identified as having been crossed over to your patients' supplemental insurers. Remittance Remark Code MA 18 will indicate your claim has crossed over to the supplemental insurer.

*        If you determine these claims were not crossed over, you are within your rights to submit claims to your patients' insurers for supplemental payment using methodologies acceptable to those entities.

Sick

So today and yesterday I took my first sick days in probably 10 years.  I don't get sick!  But my whole family had the flu and I got a touch of it too.  Still working today from home but I wanted to make sure I didn't get my coworkers sick for the Super Bowl.

Tracking/Trending of your electronic claims - Show me the money!

AFC Champs!

Congrats to the Colts on winning the 2009 AFC Championship!  We were there and it was a great game.  We're still more than a week away from the Superbowl but you can be sure there will be plenty of posts about that in the coming week from all of us here at Bradford-Scott Data Corporation.


Speaking of Bradford-Scott, there were a number of us at the game.  Our Medical Software manager sat in on the goal-line.  Our Electronic Health Records manager sat on the 20.  A long-time medical claims software programmer sat in the end zone.  And my family and I were in the corner along with our EHR lead trainer and her parents.
